FOIA Request:

A  FOIA Request Form must be filled out when requesting a copy of public documentation.  All requests will be answered within the time frame alloted by the Sate of Illinois (varies by amount/type of documentation).


Medical Record Request:
A Release of Medical Records form must be filled out by the Patient listed on the Report (If under 18 years old, by a parent or guardian) before records can be copied.  A photo id is required upon pick-up of any records.


Ambulance Billing:

RealEstate Taxes simply do not cover all of the expenses related to providing ambulance service and thus we invoice Residents and Non-Residents for the remainder of our cost.

If you have recently been transported and have not provided your insurance, Medicare or Medicaid information, please fill out the following form so that we may collect from them.

April 9, 2013 Consolidated Election

 

The Spring Grove Fire Protection District will participate in the April 9, 2013 Consolidated Election.  Two (2) trustee positions will be on the ballot.  The terms are six (6) years in length.  Nominating petitions are available at the District Station, 8214 Richardson Rd., Spring Grove, IL.  The first day to file petitions is Monday December 17, 2012.  The last day to file is December 26, 2012.
